Knee
Regular review of pertinent scientific journals always reveals more articles about anterior cruciate ligament (ACL) reconstruction than any other subject. Current issues of interest include graft choice, fixation method, graft rupture rates and revision procedures.
The bone-patellar tendon-bone graft continues to be regarded as the gold standard, but the use of hamstring graft is increasing in popularity. The reasons for this include concern about long-term residual quadriceps muscle inhibition, with some experts wondering whether this may cause a slightly increased risk of subsequent knee arthritis. However, several up-to-date studies have not demonstrated any differences in the overall results.
Shoulder
Controversy still exists regarding the most appropriate method for the treatment of acute traumatic shoulder dislocations. Young male patients are at the highest risk of developing recurrent shoulder instability. Research papers have described a rate of recurrent dislocation of 56% following non-operative treatment, and only 3% after surgical repair.
